Proposed Photograph, Audio & Video Consent for Experiential Training
[Entity] is committed to applying the same principles of mutuality, fairness, and bounded consent to its training environment that it teaches clinicians to bring to their clinical work. This consent form reflects those values.
1. Our goal and why it matters: High quality training footage makes [Entity] education better. Recordings of live training — demonstrations, role plays, group process — are among the most valuable educational materials we produce. We want to capture that footage well, and we want to do so with your genuine, informed consent.
2. What we record and when: Portions of this training will be recorded. Recording will be announced at the start of each session in which it occurs. You will never be recorded without prior notice within a session.
3. Your consent options: We ask that you indicate your preferences below. You may consent to all, some, or none. Your choices will not affect your participation, standing, or certification in any way.
☐ Internal use only — Recordings may be used for faculty review and training improvement but not distributed externally
☐ [Entity] training programs — Recordings may be used in future [Entity] Level 1, 2, or 3 training as educational demonstration
☐ Public educational materials — Recordings may be used in online courses, webinars, or presentations available outside the [Entity] training community
☐ Promotional use — Recordings may be used in marketing or promotional materials for the [Entity]
We encourage broad consent where you're comfortable; it directly supports the quality of [Entity] training for future clinicians. But we mean it when we say any choice is fine.
4. Clinical material: Recordings that include discussion of your clinical cases or client material, even if disguised, will not be used outside the training context under any consent level. This is non-negotiable and applies universally.
5. Right of review and withdrawal: You may request to review footage in which you appear before it is used externally. You may withdraw consent for any future use within 30 days of the training's conclusion. Withdrawal applies to future distribution only and is not retroactive to materials already in circulation.
6. Duration and scope: This consent applies only to the specific training sessions listed on this document and does not carry forward to future trainings without a new consent process.
7. If you prefer not to be recorded: We will work with you. Options include camera-off periods during recorded segments or positioning outside the primary recording frame for in-person components. We will never ask you to leave or limit your participation as a result of this choice.
8. How this consent is obtained: This form is completed at the start of training, after you have had the opportunity to meet your facilitators and ask questions. It is intentionally separate from registration. Signing it is not a condition of enrollment.
